Housing is Healthcare Fund by the Arizona Medicaid Health Plans - Summer/Fall 2026

The Housing is Healthcare Fund by the Arizona Medicaid Health Plans invests in affordable housing projects that include elements that connect communities and support healthier individuals, families and economies across the State of Arizona, including housing for Medicaid-eligible individuals. The Fund was developed in 2020 by a consortium of health plans, becoming the first statewide initiative aligned to the vision and values of the national Home Matters movement. 

Local Initiatives Support Corporation (LISC) Phoenix provides underwriting and other asset management services to the fund. LISC Phoenix is collecting and processing this application. All communication about your application will come from a LISC Phoenix program officer. 
Awards will be offered based on available funding. Most grant awards will be for between $100,000 and $500,000.  
Proposals must meet ALL of the following criteria: 
  • Multi-family affordable housing projects. Home ownership projects are not eligible.
  • The project must serve families or individuals earning up to 80% AMI. Mixed income properties will be considered based on the income distribution of units.
  • The number of units built or renovated must be significant to the local community.
  • Projects must be "shovel ready". Projects that have been fully or substantially completed are not eligible.
  • A substantial portion of financing must be committed and demonstrated in the application. 
  • The project site must be owned,  under purchase agreement or long-term lease. 
  • Projects must demonstrate a deliberate connection to community assets and cross-sector partnerships, demonstrated by genuine community engagement. Examples include, but are not limited to:
    • Healthy food retailers that accept food assistance programs like WIC and SNAP
    • Employment centers that match resident skills
    • Active transportation facilities and public transit
    • Schools, childcare or senior centers (where applicable)
    • Medical facilities, such as federally qualified health centers
    • Public libraries
    • Parks or community centers
    • LEED and Fitwel standards
    • Places of worship

In addition to completing all questions in the following form, you will be asked to upload the following back-up documentation: 
  • Comprehensive Community Development Plan, if applicable (form page 2)
  • Application narrative covering the following in no more than five pages (form page 6)
    • Approach to Comprehensive Community Development: Describe your organization's approach to comprehensive community development. What experience or connection does your organization and/or partners have in the neighborhood where the project will be developed? What role does resident engagement and leadership play in your work generally and in this project specifically? 
    • Addressing the Social Determinants of Health: Describe the project and the community needs it addresses. For each SDoH area you selected on Page 4, describe your approach and the experience your organization and/or your partners bring. What is your history of working together? How do you intend to document your work together on this project?
    • Serving AHCCCS Members: Describe the partnerships you have in place to proactively serve AHCCCS members. If you do not have relationships in place, how best can ABC/HOM Inc. (the AHCCCS statewide housing administrator) and other AHCCCS insurers connect their members to opportunities for housing at this project?  
  • MOUs or similar agreements with SDOH collaborators, if applicable (form page 6)
  • Construction budget (form page 6)
  • Brief construction timeline (form page 6)
  • Sources & uses statement (form page 6)
  • Proof of all committed financing (form page 6)
  • Project proforma, if applicable (form page 6)
  • Financial Statements for the most recent three years. (form page 6)
