(name of participant) understand that if accepted in the NJLISC HDTI program that the program requires (i) at minimum, attendance in 80% of the classes/sessions, (ii)active participation in class sessions(iii) utilization of the online Learning Management System (LMS) resources to catchup on missed sessions and that  non-compliance in any respect will result in elimination from the  program.

For individuals from non-profit organizations and governmental entities.  Please note that if accepted in the program your supervisor’s agreement or approval is required.  Following acceptance in the training, the supervisor’s acceptance form will be e-mailed to you for execution and submission by your supervisor.
