Thank you for taking part in LISC NY's annual partner survey. This survey collects your basic organizational information, such as the demographic profile of your board, staff, and leadership, your financial statements, and descriptions of the community you serve and programs you offer. As part of this, we ask that you upload organizational documents such as your financial statements. This information enables us to be responsive to needs and opportunities that you bring to our attention, and allows us to quickly consider your organization for any upcoming partnership or funding opportunities. After you submit this information, we may request a follow up meeting with you and key members of your team to learn more about your organization and discuss potential partnership opportunities.

 

If you responded to our survey last year, we only ask that you fill in the required fields and update any information that has changed over the last year. Please note that all information you provide in this update will remain confidential, protected in encrypted IT storage with limited access. Any data collected will only be reported out in the aggregate of all responses.

Organizational Information











There are different dimensions of identity, but for 2023 partners we are focusing on racial and ethnic identity of the primary organizational leader (executive director, CEO, majority owner). Even if you have worked with us for some time, your voluntary response will ensure that we are capturing how you yourself identify. You also have an option to share specific nationality information as ethnic and racial categorical groups are not singular and span many different identities with unique cultures.   

Information submitted here is confidential to the general public, protected within LISC IT systems, and will only be reported in aggregate.  

Your responses will help us be responsive to government and private funders, who are increasingly asking for this information and promote our own accountability to LISC’s 10x goals where we aim to scale proven solutions, seed new ideas for closing racial health, wealth and opportunity gaps and build the capacity of our partner organizations to do so.
